A bigger bet on wakefulness
Alkermes is changing shape. It used to lean mainly on marketed neuroscience drugs like LYBALVI, VIVITROL, and ARISTADA, plus older royalty and manufacturing revenue. After buying Avadel in February 2026, it now owns LUMRYZ, a once-at-bedtime oxybate for narcolepsy.
The bull case is that this move gives Alkermes a real sleep medicine business before alixorexton, its orexin 2 receptor agonist, could reach the market. LUMRYZ had about 3,600 patients on therapy exiting the quarter, and it added $39.5 million of sales in a partial Q1 2026 period. If the REVITALYZ study in idiopathic hypersomnia works, LUMRYZ could add another label.
The pipeline story is also getting wider. Alkermes is starting ALKS 7290 studies in adult ADHD and preparing ALKS 4510 for fatigue tied to multiple sclerosis and Parkinson's disease. That makes orexin look more like a platform than one sleep drug, but the proof still has to come from larger trials.
The bear case is balance sheet timing. Alkermes took on about $1.525 billion of debt for Avadel. It also faces possible Teva generic VIVITROL entry in early 2027, just as it is funding Phase 3 alixorexton work and new mid-stage trials.
Selling today, funding trials for tomorrow
Alkermes makes money mostly by selling its own medicines. In Q1 2026, proprietary product sales were $338.1 million. VIVITROL contributed $112.4 million, ARISTADA family products $93.8 million, LYBALVI $92.4 million, and LUMRYZ $39.5 million after the Avadel deal closed on February 12.
The smaller revenue stream is manufacturing and royalty revenue. It was $54.8 million in Q1 2026, down from $62.0 million a year earlier, mainly because RISPERDAL CONSTA manufacturing revenue fell. This line is not the future growth story.
The model works if marketed products keep producing enough cash to pay debt service and fund trials. LYBALVI growth helps. LUMRYZ growth helps. But if VIVITROL falls faster than planned or LUMRYZ adoption stalls, the company has less room to spend on orexin studies.
